2. Nature of Counseling
Counseling services are provided to individuals, couples, and families facing challenges such as communication issues, relationship difficulties, anxiety, depression, and other concerns. Counseling involves working collaboratively to develop goals and strategies for change. The therapeutic process is tailored to each client’s unique needs and goals.

3. Privacy
All sessions are confidential, except in cases where disclosure is required by law, such as concerns about safety or abuse.

4. Fees and Payment
Fee schedule: 90791 – $300; 90834 – $200; 90837 – $250; 90847 – $250; 99050 – $30.
Payment is due at the time of service. Accepted forms of payment include credit card and HSA card.

5. Complaints
If you have concerns about the services provided, you are encouraged to discuss them with me directly. You also have the right to file a complaint with the Ohio Counselor, Social Worker, and Marriage and Family Therapist Board:
